PREÇO POR SEMANA:

GENERAL PUBLIC: 155€, or 140€ for siblings*

SÓCIOS NOVAS DESCOBERTAS** : 130€, ou 115€ para irmãos*

*The sibling discount is only valid within the same tax household.

2. PARTICIPANT'S MEDICAL INFORMATION

Please indicate if the participant has any allergies/intolerances, follows a specific diet, takes medication or has any other relevant health condition. In the event of any of these situations, the respective prescription or medical statement must be presented. It is the responsibility of the parents or legal guardian to ensure that all the information provided is true and complete and that the vaccination record is up to date.
NOTE: It is recommended that the participant be assessed by their doctor before attending any New Discoveries Summer Camp. By signing this form, the guardian declares that the participant is in good physical and psychological health to take part in the camp.
All participants in the New Discoveries Summer Camps are covered by personal accident insurance for the 24 hours of the days in which they take part in the camp, under the terms of article 17 of Decree-Law no. 304/2003, of December 9, and Ministerial Order no. 629/2004, of June 12.

The participant meets the necessary conditions to take part in the Summer Camp organized by the Novas Descobertas Association, including the practice of sporting activities. I, as guardian, declare that I have read and understood the rules set out in the Internal Regulations of the Novas Descobertas Summer Camps.
The participant's guardian must sign the Term of Responsibility that will be sent via email in order for the participant's registration to be valid.
